LGV PCV Taxi Medical

When you apply for your provisional CAT C or CAT D licence, you will need to enclose a Medical Report. This is on a standard form – D4 which has to be completed by a doctor. Any doctor can do this for you but to save the hassle of making appointments we offer this service at our HGV training centre on alternate Saturday mornings. If you are not within reasonable travelling distance of our training centre, simply call us to arrange a LGV medical local to you. For LGV/PCV licence it’s purely a commercial transaction and your medical records are not needed. For Taxi medicals the Doctor will require a basic GP summary.

We keep all the necessary D2 D4 forms at the centre and will gladly assist you in completing the licence application.
If you wear glasses or contacts please be aware the doctor will ask you to take out your lenses;  you will need to bring along a recent prescription for your glasses/lenses.

The medical service is open to any new driver or drivers needing a renewal or Taxi medicals.
